Terms and Conditions of Service

1. Introduction

These Terms and Conditions govern the professional services provided by Fundamental Health Ltd (“the Practice”).

By booking an appointment, you confirm that you have read, understood and agree to these Terms and Conditions.


2. About the Practice

Fundamental Health Ltd provides independent online mental health assessments and interventions for adults aged 18 years and over.

Services are provided by Debra Dew, a Registered Mental Health Nurse regulated by the Nursing and Midwifery Council (NMC).


3. Scope of Service

The Practice provides:

  • Comprehensive mental health assessments
  • Brief interventions
  • Mental health advice within the clinician’s professional scope of practice

The Practice does not provide:

  • Emergency or crisis intervention
  • A 24-hour service
  • Psychiatric diagnosis or prescribing (unless this service is introduced in the future)
  • Reports for legal proceedings unless agreed in advance
  • Fit notes or sickness certification

4. Eligibility

Clients must:

  • Be aged 18 years or over.
  • Be able to provide informed consent to treatment.
  • Reside within the United Kingdom unless agreed otherwise.

5. Appointments

Appointments are delivered securely online.

Clients are responsible for:

  • Having a suitable internet connection.
  • Joining appointments on time.
  • Ensuring they are in a private and confidential location.
  • Minimising interruptions during the consultation.

6. Fees

Current fees are published on the Practice website.

Payment must be made in advance of each appointment unless otherwise agreed.

Appointments will normally only be confirmed once payment has been received.


7. Cancellation Policy

Appointments may be cancelled or rearranged without charge provided at least 48 hours’ notice is given.

Where less than 48 hours’ notice is provided:

  • The full appointment fee may be charged.

If the Practice needs to cancel an appointment, an alternative appointment will be offered or a full refund provided.


8. Late Attendance

If you join your appointment late, the session will usually finish at the agreed time.

Where more than 15 minutes late, the appointment may be treated as a missed appointment and the full fee may be charged.


9. Missed Appointments

Failure to attend an appointment without prior notice will normally result in the full appointment fee being payable.

Repeated non-attendance may result in discharge from the service.


10. Confidentiality

Everything discussed during appointments is treated confidentially.

However, confidentiality may be breached where there is:

  • Serious risk of harm to yourself.
  • Serious risk of harm to another person.
  • Concerns regarding abuse or neglect.
  • A legal obligation to disclose information.
  • A court order requiring disclosure.

Where possible, this will be discussed with you before information is shared.


11. Safeguarding

Fundamental Health Ltd has a legal and professional duty to safeguard adults and children.

If concerns arise regarding abuse, neglect or serious risk of harm, information may be shared with relevant statutory agencies in accordance with legal and professional responsibilities.


12. Emergencies

Fundamental Health Ltd is not an emergency service.

If you are experiencing a mental health crisis or believe you are at immediate risk of harming yourself or someone else, you should:

  • Call 999 if there is an immediate danger.
  • Contact NHS 111.
  • Contact your GP.
  • Contact your local NHS Mental Health Crisis Team.

13. Clinical Records

Accurate clinical records will be maintained for every appointment.

Records are stored securely in accordance with UK GDPR and the Data Protection Act 2018.

14. Communication

Routine communication may take place by:

  • Email
  • Telephone
  • Secure online consultation platform

Email should not be used for urgent clinical matters.

The Practice aims to respond to routine enquiries within two working days.


15. Professional Boundaries

The client-practitioner relationship is a professional one.

The Practice cannot accept requests through personal social media accounts and cannot provide advice through text or social media messaging.


16. Ending Engagement

Engagement may end:

  • By mutual agreement.
  • At your request.
  • When individual goals have been achieved.
  • If the service is no longer clinically appropriate.
  • Following repeated missed appointments.
  • If behaviour towards the clinician is abusive, threatening or inappropriate.

Where appropriate, recommendations for alternative support will be provided.


17. Complaints

If you are dissatisfied with any aspect of the service, you are encouraged to discuss this with the Practice as soon as possible.

A copy of the Complaints Policy is available on request and on the Practice website.


18. Changes to Terms and Conditions

Fundamental Health Ltd reserves the right to amend these Terms and Conditions.

The most recent version will always be available on the Practice website.


19. Governing Law

These Terms and Conditions are governed by the laws of England and Wales.


Client Declaration

By booking an appointment with Fundamental Health Ltd, I confirm that:

  • I have read and understood these Terms and Conditions.
  • I understand the limits of confidentiality.
  • I understand that this is not an emergency or crisis service.
  • I agree to the Practice’s Privacy Notice.
  • I agree to the cancellation policy and payment terms.
  • I consent to receiving services from Fundamental Health Ltd.
